Medical Expenditure Panel Survey - U.S. Department of Health and Human Services, Agency for Healthcare Research & Quality. Detailed data on insurance premiums, employee co-pays by state, industry, employer size (number of workers), ownership and age of firm. Hundreds of pages of data for 1978-2005. Updated in late July each year. 2006 data will be available July of 2008. The Medical Expenditure Panel Survey is an annual set of large-scale surveys of families and individuals, their medical providers (doctors, hospitals, pharmacies, etc.), and employers across the United States. MEPS collects data on the specific health services that Americans use, how frequently they use them, the cost of these services, and how they are paid for, as well as data on the cost, scope and breadth of health insurance held by and available to U.S. workers.

Kaiser Family Foundation Employer Benefits Survey (annual) From company web site:
“…trends in employer-sponsored health coverage, including changes in premiums, employee contributions, cost-sharing provisions…” Survey of approximately 2,000 companies. Researchers at the Kaiser Family Foundation, the National Opinion Research Center at the University of Chicago, and Health Research and Educational Trust design and analyze the survey.
